Eyebrow hair transplant in frontal fibrosing alopecia (FFA) procedure is not something unique but is similar to the usual hair transplant process. The FFA is a form of scarring hair loss and eyebrow hair loss is the primary feature of it. It usually disturbs & creates significant distress to patients. However hair transplant surgery is the common option of treatment in such cases but it might not be a permanent solution. While, it is considered as cosmetic technology where grafts are transferred to the patient’s eyebrow area. In this way new hairs will grow to make up for the lost hair, yielding the normal look of face & eyes.
The eyebrow hair transplant in FFA is carried out by common Follicular Unit Extraction (FUE) technique, in practice in restoring the hair. But prior to eyebrow hair surgery, there are multiple aspects to keep in mind for risk and side effects. It is a minimally invasive procedure and is comprehensively effective in providing patients, aesthetically pleasing results.

The FUE procedure starts with creation of a very small / microscope level incision at places in the eyebrow where hair grafts are to be inserted. It is very vital & critical for surgical specialists to keep the natural angle and curvature of grafted hairs in such a way that it creates results close to nature and realistic in appearance. The number of hairs to be transplanted varies from patient to patient and the density needed at the recipient site. The surgeon transfers not just the extracted single hair but also follicles too. By this way chances of new hair regret is fairly bright. The entire transplant process takes just two to three hours. Usually 250 to 350 numbers of follicles are removed from the donor site & transplanted in an eyebrow.

The hairs for grafting are harvested from non-affected follicles of occipital scalp skin. It is either the back or sides of the scalp. However, if donor hairs are not healthy over the scalp area, then other parts of the body can also be used to harvest follicles for eyebrow grafting.

After the surgery, it takes 3 to 6 months to start growing new hairs. And after another three to four months, patients see complete results on the eyebrow.

The eyebrow hair transplant in FFA has been found very successful in the short term, meaning grown hairs stay satisfactory for two years or so but unsatisfactory in the long term. Because after 2 / 3 years, restored hairs start falling successively in the next four to five years. Only fewer patients who got grafting of eyebrow hair but have no complaints even after five years. One can admit that surgical procedures incorporated in eyebrow hairs transplants had successes because some patients got excellent benefits for shorter time while few others are enjoying restored hair in the long run.

Doctors had derived to some extent that fall of the eyebrow hairs have many reasons and it may be due to one or combinations of more than one of following reasons:

·         Skin issues

·         Inflammation and destruction of hair follicles

·         Internal health issues

·         After effects of chemotherapy

·         Any physical issues like chemical reactions of makeup administering or trauma to the area etc.

Therefore, now the consultant doctors have made it routine to refer all such complaining patients of falling hairs after restoration procedure either in short  or long term to a Board consisting of dermatologist, cosmetic & transplant surgeons to sort out the real cause of hair fall after surgery. The Board interacts with patients in detail for prior & post operation health scenarios and tries to diagnose causes and evaluate features behind the hair fall with passage of time. If needed, the patient is recommended for in depth investigations through pathological tests as well as biopsy. The investigation is being used to contain or slow down the postoperative rate of hair fall.
 
Caution & Limitations

·         Patients need to avoid vigorous exercise for four weeks after the surgery.

·         Trimming of transplanted hair is not recommended before one month of surgery.

·         Patients are informed in advance about the success ratios of the surgery.
